Job Description:

The Director of Case Management will coordinate the integration of social services and case management functions into patient care, discharge, and home planning processes. They will collaborate with various departments, external organizations, and healthcare facilities to ensure seamless care delivery.
Responsibilities include but are not limited to:
- Mobilizing resources and conducting interviews to achieve desired clinical outcomes within the expected timeframe.
- Ensuring appropriate utilization of services from admission through discharge.
- Evaluating patient satisfaction and quality of care provided.
- Acting as a patient advocate, investigating and reporting adverse occurrences, and educating staff on resource utilization, discharge planning, and psychosocial aspects of healthcare delivery.
- Conducting concurrent medical record reviews using approved indicators and criteria.
- Facilitating interdisciplinary patient care rounds or conferences to review treatment goals, optimize resource utilization, provide family education, and identify post-hospital needs.
- Developing and executing care plans and achieving goals in collaboration with clinical staff.
- Coordinating social services to assist patients, families, and significant others in coping with illness and maximizing healthcare benefits.
Must-Haves:
- Graduate of a Nursing program at the Bachelor's of Science level or higher
- BLS certification required upon hire
- Arizona Registered Nurse and/or Compact State Registered Nurse required
- Minimum of 2 years as Director of Case Management in a critical care hospital
Nice-To-Haves:
- CCM or ACM Certification preferred
- Master’s degree preferred
- 5 years in Case Management leadership or leadership in critical care hospital upper management

Candidate Details:
This newly created position requires in-person reporting at a specific location. Our ideal candidate is a graduate of a Nursing program at the Bachelor's level or higher, with a minimum of 2 years of experience as Director of Case Management in a critical care hospital. Certifications in BLS and Arizona or Compact State RN licensure are mandatory. Preferred candidates will have CCM or ACM Certification, a Master’s degree, and 5 years of leadership experience in Case Management or critical care hospital management.
