Overview:
We are seeking a compassionate and skilled Hospice Nurse to join our dedicated team. As a Hospice Nurse, you will provide specialized care and support to patients and their families during end-of-life stages. Duties:
- Conduct comprehensive assessments of patients' physical, emotional, and spiritual needs
- Develop and implement personalized care plans in collaboration with the interdisciplinary team
- Administer medications and treatments as prescribed
- Monitor patients' condition and provide ongoing evaluation and support
- Educate patients and their families on disease progression, symptom management, and end-of-life care options
- Coordinate with other healthcare professionals to ensure continuity of care
- Provide emotional support and counseling to patients and their families
- Document patient care activities accurately and maintain up-to-date medical records
Experience:
To be successful in this role, you should have the following experience and skills:
- Minimum of 2 years of experience as a Registered Nurse (RN)
- Experience in hospice or palliative care is preferred
- Proficient in diagnostic evaluation and assessment
- Knowledge of dementia care principles and techniques
- Ability to triage patient needs effectively
- Competent in medication administration and management
- Proficient in maintaining accurate medical records
