Our work environment includes:
LVN responsible for providing case management and care coordination support for assigned and complex specialty referrals cases as well as post hospitalization coordination of care. Assures that all specialty and ancillary referrals are processed and completed in a timely manner. Responsible for submitting monthly department reports to clinic administrators. Work effeciently with the clinical team and IPA/HMO and specialist network representatives to coordinate appropriate patient level of care.
1. Provide case management for assigned cases.
2. Supervision of referring department including monitoring assignments and performance, coordinating and providing team coverage as needed, as well as providing referrals staff training and evaluations.
3. Responsible for assuring assigned referrals are completed in a timely manner. Assures all assigned urgent referrals are processed within 48 hours.
4. Responsible for working closely with the Clinical Team, IPA/HMO and specialist network representatives to coordinate appropriate patient level of care.
5. Responsible for monitoring and tracking incoming consultation reports to ensure PCP received and signed-off on reports, and that any findings requiring follow up have been completed.
6. Responsible for preparing, assisting, and submitting monthly and quarterly data to administration.
7. Responsible for collecting and reporting data and other information as requested by IPA/HMO and other agencies. Provides reports of progress to administration.
8. Responsible for using standards of documentation and maintaining and enforcing confidentiality.
9. Responsible for providing orientation to new providers and staff on referral system.
10. Responsible for other related duties as assigned by administration.
Requirements:
1. Experience in family medicine setting.
2. Certified medical assistant.
3. Case Management, Referrals and/or Managed Care experience preferred.
4. Computer skills, including Microsoft programs and knowledge of Electronic Medical Records systems.
5. Bilingual English/Spanish
Job Type: Full-time
Pay: $16.90 - $18.00 per hour
