Job Overview:
Provide support to Day Habilitation Director and Assistant Director in programmatic oversight of Day Habilitation Service. Oversight of case management for Day Habilitation Service. Facilitate development and implementation of Day Habilitation Service Plans (DHSP) with the interdisciplinary team, representing the interests and needs of the people served. Working knowledge to develop and implement CARF standards, DDS standards and any other standards related to Long Term Services and Supports (LTSS). Develop and maintain a working relationship and communication with families, residences, providers and others involved in the care of the person served. Perform other related work duties as assigned.
Essential Functions of Position:
- Work with the Director and Assistant Director of Day Habilitation in the planning, development and implementation of agency goals and objectives.
- Ability to communicate and work effectively with all levels of personnel.
- Ensure compliance with CARF standards, Mass Health EEOHS, DDS, agency standards and any additional statutes and regulations as it pertains to Day Habilitation.
- Perform duties to reflect agency/program policies and mission statement.
- Oversight of Case Managers within all aspects of DHSP, ISP, HCSIS, MMIS and any other electronic filing systems.
- Attend all Day Habilitation Service Plan (DHSP) meetings to support the person served.
- Facilitate person-centered program development, including identification of possible adaptive equipment and providing referrals to attain adaptive equipment.
- Ensures ISP required assessments, documentation, reports and other relevant information is correctly entered into HCSIS as needed.
- Assist individuals served in the development and articulation of their needs through the DHSP process, program, family and other providers.
- Assist with day to day operational duties and responsibilities of Day Habilitation as assigned by Day Habilitation Director and Assistant Director.
- Maintain an excellent working relationship with state funding agents, community resources, other human service agencies and individual families.
- In collaboration with Director of Day Habilitation and Assistant Director coordinate and conduct regularly scheduled Case Management meetings
- Act as an advocate for people we serve.
- Observe goal facilitation by direct care staff; training of direct care staff to ensure implementation of the Day Hab Service Plans and completion of progress notes; including all documentation and maintenance of records to demonstrate compliance with funding source requirement.
- Maintain case records including DHSPs, Safety Assessments, Service Needs Assessments, Monthly and Semi-Annual Reports and other documentation according to MassHealth, EEOHS and DDS regulations.
- Provide support to individuals with physical challenges, such as: lifting, transferring and ADL’s.
- Facilitate consumer-centered program development, including identification of possible adaptive equipment and providing referrals to attain adaptive equipment.
- Assist each individual in the development and articulation of their needs and desires through the DHSP process, program, family and other providers.
- Lead Day Habilitation PBS initiative.
- Review consumer progress to determine appropriateness of services.

EDUCATION and/or EXPERIENCE
Bachelor's degree in human services or related field required and at least 2 years of experience in a case management capacity working with individuals with developmental disabilities.
Demonstrated ability to work both independently and collaboratively with a culturally diverse population of individuals, co-workers, care providers, family, community members and others.
Ability to write clear, concise and accurate documentation.
Valid driver’s license and acceptable driving record is required.
Current First Aid, CPR and Crisis Intervention certificates or be able to obtain these within thirty (30) days of hire. Pass criminal background check (CORI).
HCSIS experience is preferred.
Job Type: Full-time
Pay: $22.25 - $24.00 per hour
