Job Purpose/Summary
The Supportive Living Specialist provides skill-building services to individuals with Serious and Persistent Mental Illness (SPMI) residing in the community using the recovery model, which encourages independent choice and empowerment. Duties include training, educating, and assisting consumers in daily living skills, community integration, medication compliance, apartment/household maintenance, self-care skills, and independence skills, which are implemented in an Individualized Service Plan (ISP). The position provides direction and assistance to individuals regarding their psychiatric, medical, social, and programmatic needs. Additional duties include linkage to services, collaboration with the treatment team members, staff in-service training, and quality assurance monitoring.
Minimum Qualifications
Education: Bachelor’s degree in a social services related field.
Experience: Minimum of 1 year of experience working in a mental health or social welfare agency required.
Licenses, Certifications & Clearances:
· Act 33/34 Clearances
· FBI Clearance
· Valid State of PA Class 1 Drivers License
· Access to properly insured motor vehicle for client transportation and fieldwork and a safe driving record

Essential Duties & Responsibilities
· Assures provision of quality skill building services for consumers by:
o Providing skill building services to consumers:
§ Assists in the development and implementation of the ISP
§ Trains and educates in completing daily living skills, such as personal hygiene, apartment management, shopping, public transportation, financial management, tenant responsibilities, and general home maintenance activities and related repairs
§ Provides on-going emotional support and intervention for individuals with SPMI
§ Monitors and assists with individual’s psychiatric and medical needs, including accompanying to appointments, when necessary, and encouraging follow-up with medical orders
o Completing documentation:
§ Maintains clinical records. Charts contacts with consumers and collaterals, updates forms, as needed, and completes all necessary paperwork, including daily service activity logs and daily charting of progress towards ISP goals
o Administrative duties:
§ Participates in Mission, Vision, Philosophy (MVP) teams and panels
§ Communicating with, referring to, and coordinating services with other WFS staff, non-agency personnel, the community, and other related services
o Continuation of professional growth and competency:
§ Participating in individual supervision, which is focused on seeking feedback and receiving guidance
§ Attending staff in-services, trainings, and external conferences
§ Participating in staff meetings
§ Reading/Reviewing recommended literature and education materials
· Contributes to the effective operations of the program by:
o Completing required documentations in individual case records, including progress notes for direct and indirect services, encounter form documentation, periodic updates, incident reports, ISPs/reviews, and memo to charts. Maintaining a case record by following chart organization and ensuring accuracy of information by verifying specific information in the Credible system. Completing vehicle mileage logs and accident reports
o Completing attendance reports, training records, and vacation requests according to policy, and following call off procedures. Attending staff meetings and trainings, as scheduled, and communicating with peers and supervisory personnel effectively
o Addressing all issues of fraud, waste, and/or abuse as they arise, and proactively looking for ways to prevent instances of fraud, waste, and abuse in the service delivery system and work systems of the agency as described in the agency Compliance Plan
