Hospice by Loving Care is the Hospice division of Loving Care In-Home Health Services. We are a home health, hospice and private duty services provider since 1997. Our health care services are available to people in 16 counties throughout central Oklahoma.
This position will do visits out of the Noble Hospice branch location. The Hospice RN Case Manager position is responsible for the provision and documentation of hospice services to patients in accordance with an established interdisciplinary plan of care. This position will complete visits in Noble, Norman, Moore and OKC metro surrounding areas. This position requires the following:
- Manages and coordinates all aspects of patient care to assigned hospice case management patients;
- Responsible for providing skilled nursing assessments and evaluation; planning and patient care according to physician orders;
- Participates in Interdisciplinary group meetings;
- Instructs caregivers and family members on disease process and symptom management;
- Ensures implementation of activities for improved quality of patient care;
- Supervision of Hospice care team members to include LPNs, Home Health Aides, Chaplains, Social Workers, etc.;
- Ensuring patients needs such as comfort measures, pain management, psychosocial, spiritual needs, volunteer and/or bereavement issues are met.
Qualifications and Skills:
- Active Registered Nurse license in State of Oklahoma without restrictions;
- Prefer applicant has previous hospice nursing experience, but not required;
- Current CPR certification;
- Valid current Drivers License;
- Current required Automobile Insurance requirements.
