Responsibilities:
-Understands the philosophy and mission of hospice and palliative care
- Provide direct patient care under the supervision of a registered nurse or physician
- Administer medications and treatments as prescribed by healthcare providers
- Monitor and record patient vital signs, symptoms, and reactions to medications
- Assist with diagnostic tests and procedures
- Collaborate with healthcare team members to develop and implement patient care plans
- Educate patients and their families about medical conditions, treatment plans, and self-care
techniques
- Maintain accurate and up-to-date medical records
Experience:
- Minimum of 1 year experience as a Licensed Practical Nurse (LPN)
- Strong knowledge of human anatomy, physiology, and medical terminology
- Proficient in using medical equipment and tools
- Experience in providing care for patients with diverse needs
- Familiarity with accurate documentation and billing purposes
Skills:
- Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare team members. Must be able to communicate verbally, visually, and in writing
- Strong critical thinking and problem-solving abilities to make sound clinical judgments
- Ability to work independently as well as collaboratively in a fast-paced healthcare environment
- Attention to detail to ensure accurate documentation and medication administration
- Physical stamina to lift, move, and position patients as needed
