Square Medical Group is looking for energized individuals who want to join our very supportive team to build an exciting new Low Threshold Residential program in Quincy! This 32-bed program will provide Housing First lodging, a drop-in center, and case management services for adults in downtown Quincy!
Agency Summary: Founded in 2008, Square Medical Group is one of Massachusetts’ leading providers of comprehensive behavioral health services. We offer a wide range of services for mental health and substance use disorders with a continuum of care that includes medication management, individual and group therapy, IOP, PHP, residential services, and TMS.
Position Summary: Located near a MBTA bus stop, the Copeland Houses are a Low Threshold Transitional Housing program. The smaller of the two houses consists of three suites for couples and seven female SRO's (single room occupancies) while the larger of the two houses consists of twenty-two male SROs with shared bathrooms. These houses are case management focused and based on the Harm Reduction model.
The goal is to place our residents in permanent housing that allows them to improve their quality of life. Being able to identify everyone’s barriers to housing is an important part of this role.

You will also be able to maintain a non-judgmental, compassionate approach while keeping consistent boundaries with our residents. This is a part time position, 20 hours per week scheduled for Saturday and Sunday 8:30a -4:30p full time, and one weekday part time (4hours) to be coordinated with the supervisor!
Qualifications & Education:
- High School Diploma or Equivalent certificate required.
- Bachelor’s degree in a human service-related discipline preferred.
- LADC 2 certificate preferred.
- Previous detox and or CSS (Community Support Service) experience preferred.
- Understanding of Social Determinants of Health preferred.
- Knowledge of community resources in the Greater Boston Area including but not limited to housing assistance, education assistance, primary and medical care, vocational assistance, transportation assistance, and other social services preferred.

- Ability to quickly learn and integrate new information.
- Good computer skills such as: Word, Excel, Outlook, Video conferencing.
Essential Functions & Responsibilities:
- Articulate with good communication skills.
- The Case Managers will be able to take direction, be prompt, take initiative, complete tasks as assigned and be able to work effectively as part of a team.
- Assessment of each resident’s case management needs including behavioral health, housing, financial, health and wellness, education and training, vocational, social.
- Planning: in collaboration with other house staff, develop and implement a plan to address the needs noted in the assessment.
- Linkage: link the resident with any other staff or services, including those outside the house and outside the Square Medical Group, to achieve the goals of the plan.
- Monitor the progress of the resident and adjust the plan as needed.
- Advocate for the resident to achieve and utilize necessary services.
- Support the client throughout the case management process utilizing elements of Motivational Enhancement and a strength-based approach.
- Engage in outreach activities to identify necessary resources.
- One on one coaching and support to house residents.
- Support residents’ participation in recovery-oriented activities.
- Helps residents identify and access needed resources and recovery supports.
- Discharge planning including interface with other Square Medical Group and community resources.
- Participate in staff, community and program meetings.
- Collect data and complete reports in a timely manner.

- 1. Sick Time (1 hour accrued for every 30 hours worked, maximum 40 hours per year).
