Seeking a Job Coach to join our Supported Employment Program (SEP) Team!
Major responsibilities are to provide on-site training and supervision of participants working in the community and to assist in learning and completing tasks assigned to them. The Job Coach works directly with the participants served so that all goals and objectives, procedures, time frames and responsibilities assigned involve the active participation of the participant. Ensuring a successful outcome for the participants by collaborating with employers and family members and provide accommodations as needed. The job coach participates in scheduled staffing, record keeping and in the evaluation of the participant's performance.
Qualifications:
- Undergraduate degree in social services is preferred.
- 2 years of relevant experience with Adults with Intellectual/Developmental Disabilities.
- Supervisory experience required.
- Previous Job Coach experience is a plus.
- Valid Illinois driver's license required. Must be insurable under Oak Leyden's vehicle insurance and have own transportation.
- Ability to maintain accurate and timely records of worker’s performance on the job and complete all reports, records and forms required.
- Be available and able to work with participants at the time scheduled for the participant's job, which may not coincide with regular workday hours.
