Position Description: To ensure the health and safety of members by performing required member visit and update for a Significant Change Event Assessment, member update visits and phone calls by speaking with the member, family member and or any other involved parties to determine member’s current status and needs.

Your duties will include:
- Perform Significant Change Event assessment visit for members who have had a change in their living arrangements due to a life crisis or significant health event
- Utilize clinical judgement and critical thinking skills to facilitate appropriate filing of incident reports in the Incident Reporting Management System with contractual required timeframes and complete appropriate follow up based on the incident
- Work collaboratively with area hospitals, nursing facilities, assigned care managers and member support teams to ensure a fluid transition to the community by identifying and aligning appropriate community supports
- Perform contractual SCE follow up- 24-hour phone call, obtaining discharge paperwork for the hospital and or nursing facility, communication and follow up with PCP, work closely with assigned COA CM to ensure continuity of care back to the community
- Travel within a 5-county region to perform post discharge assessments
- Develop and implement a process to support field-based care managers in assisting member with submitting the correct information to their PCP to request durable medical equipment
- Act as an advocate for members with service providers and others, as needed
Qualification Requirements:
- Ohio licensed Social Worker or Registered Nurse in good standing with the credentialing board.
- One year of experience working with persons with disabilities/chronic conditions and LTSS.
- Prior or current experience with mental/behavioral health populations.
- Prior experience completing electronic documentation.
- Prior case management experience and experience working in a program with strict and multiple contractual requirements is a plus.
- A good working knowledge and understanding of issues related to the elderly population.
- Knowledge of the individual, family, social and the individuals functioning in the aging process.
- Knowledge of community resources within Southwestern Ohio.
- Good customer service skills.
- Good interpersonal communication skills.
- Ability to adapt to frequent change.
- Ability to collaborate and problem solve in a self-directed team environment.
- Good oral and written communication skills.
- Good organizational and teamwork skills.
- A valid driver’s license and reliable transportation for travel within Butler, Clermont, Clinton, Hamilton and Warren Counties.
- Proficient computer skills including the Windows operating systems, MS Word, MS Excel, data entry and the ability to learn new software applications as needed.
- Ability to travel within the state of Ohio for training.
