About the Role:
We are seeking a highly skilled Physical Therapist to join our team as a PT for Home Health. In this role, you will be responsible for providing exceptional care to patients in their homes, helping them to regain their strength and independence. You will work closely with physicians, nurses, and other healthcare professionals to develop and implement individualized treatment plans that meet the unique needs of each patient. Your ultimate goal will be to help patients achieve their maximum potential and improve their quality of life.
Minimum Qualifications:

Current and unrestricted Physical Therapy license in the state of California.

Graduate of an accredited Physical Therapy program.

Minimum of 1 year of experience as a Physical Therapist.

Excellent communication and interpersonal skills.

Ability to work independently and as part of a team.
Preferred Qualifications:

Experience in home health or a related field.

Experience working with geriatric patients.
Responsibilities:

Conduct comprehensive evaluations of patients' functional status and develop individualized treatment plans.

Provide skilled physical therapy services to patients in their homes, including therapeutic exercise, gait training, and manual therapy.

Educate patients and their families on the proper use of equipment and exercises to promote independence and prevent injury.

Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated and effective care.

Maintain accurate and timely documentation of patient progress and treatment plans.
Skills:
As a Physical Therapist for Home Health, you will utilize your expertise in anatomy, physiology, and kinesiology to help patients regain their strength and mobility. You will also use your excellent communication and interpersonal skills to build rapport with patients and their families, and collaborate with other healthcare professionals to ensure coordinated and effective care. Your ability to work independently and as part of a team will be essential in providing exceptional care to our patients. Additionally, your experience in home health or a related field, working with geriatric patients, and being bilingual in English and Spanish will be highly valued.
