Position Overview
Shelter House is a community of residents, staff and volunteers working together to provide basic human services
for the men and women of the Greater Cincinnati area who are experiencing homelessness. The Substance Abuse
Care Coordinator is responsible for coordinating care to Medically Assisted Treatment (MAT) clients and other
appropriate levels of care based on AOD health assessment by developing, monitoring and evaluating
interdisciplinary medical and behavioral health care plan.
Key Responsibilities & Expectations
1. Orients and educates patients about MAT process
2. Monitors the delivery of care with the interdisciplinary team.
3. Evaluates the outcomes of patient care.
4. Facilitates CD educational groups
5. Follow best practices and complete other assignments as directed by the supervisor.
6. Meet with client’s after assessment is completed by recovery counselor and discuss appropriate referrals
and services based on assessment
7. Enroll clients in Medicaid and have proper ID documents
Qualifications & Requirements
Must have one of the following :CDCA, Certified Peer Recovery Supporter (CPRS) , Licensed Social
Worker (LSW)
- Knowledge and experience with homelessness, mental health and/or substance abuse and community
resources.
- Must pass criminal background check.
- Must be able to work the required schedule of 11am-7pm
- Since the Substance Abuse Care Coordinator is responsible for driving clients in their personal vehicle,
they need to have the following: Valid driver’s license with good driving record, daily access to private
insured vehicle during work hours and the ability to be approved by Shelter House insurer.
Skills & Abilities
- Ability to function effectively in a high energy environment.
- Ability to work independently and work flexible hours.
- Excellent written and verbal communication skills.
- Possess self-initiative and demonstrated attention to detail.
- Computer literacy with MS Office Suite. Experience with VESTA is a plus.
Physical Requirements (% of time spent performing requirements)
- Time sitting- 50%
- Time spent walking- 50%
- Time spent on telephone and computer- 10%
