Job Description:
- Provide strong leadership to the nursing team, fostering a positive and collaborative work environment
- Develop and maintain staffing schedules to ensure adequate coverage and efficient patient care delivery
- Collaborate with the interdisciplinary team to develop and implement individualized care plans for residents
- Monitor the overall health and wellbeing of residents, ensuring that appropriate medical interventions and treatments are provided
- Conduct regular rounds to assess patient conditions, address concerns, and identify opportunities for improvement
- Implement and monitor quality assurance programs to ensure compliance with state and federal regulations, as well as facility standards
- Participate in regular audits, reviews, and surveys to maintain a high standard of care and service delivery
- Analyze data and performance metrics to identify trends and implement strategies for continuous improvement
- Maintain open lines of communication with residents, their families, and the interdisciplinary team to ensure that care plans are well-coordinated
- Collaborate with physicians, therapists, and other healthcare professionals to optimize resident outcomes
- Act as a liaison between the nursing team and facility leadership, conveying information, updates, and concerns as needed

Qualifications:
- Current New York State license as a Registered Professional Nurse
- Bachelor's degree in nursing (BSN) preferred.
- Proven experience in nursing leadership or management roles within a SNF or similar healthcare setting
- Strong understanding of long-term care regulations, standards, and best practices
- Excellent communication, interpersonal, and problem-solving skills
- Ability to work collaboratively within an interdisciplinary team.
- Proficiency in using electronic health record (EHR) systems and other relevant healthcare software
- Demonstrated commitment to providing compassionate and patient-centered care
Work Schedule:
7:00 am – 4:00 pm; 4:00 pm – 12:00am
