JCCA is seeking qualified Case Managers to join our Residential Treatment Campus, offering a safe and supervised setting for at risk youth, ages 5-21. The Residential Treatment Campus is located in Pleasantville, NY and has (3) distinctive programs which provide intensive support to children with severe emotional and behavioral challenges. The (3) Residential Treatment Programs are as follows:
Pleasantville Cottage School - serves children with long-term emotional and behavioral challenges
Edenwald Center – serves children who have low IQs, autism and other developmental disorders
START - serves children who need assessment, residential treatment and intensive intervention for a short period of time
Primary Job Duties:
- Participate in family conferences (Family Team Conferences, SPR’s, IR’s, etc).
- Assist with supervising family and/or sibling visits on campus or in client home when necessary; Assist in developing visiting plans with families
- Coordinate upon admission a QI assessment for all new admissions
- Collaborate with Community providers to ensure timely discharge from Qualified Residential Treatment Program (QRTP) setting
- Support Clinical and Child Care team with crisis intervention using trauma informed care
- Collaborate with team members to generate individualized case management, treatment and discharge plans.
- Responsible for arranging, participating, and documenting meetings at school at least two times per academic year.
- Implementation of Absent Without Consent (AWOC) and Commercial Sexual Exploitation of Children ( CSEC) protocols in accordance with JCCA and regulatory requirements
Advocacy to Facilitate Permanency:
- Act as liaison with NYS regulatory agencies - ACS, DSS, CSE, and OCFS - as well as officers of the court.
- Represent team in Family Court Hearings and bring feedback from court to team for follow-up.
- Attend QRTP court hearings and provide report outs
- Refer and follow-up with community resources as needed.
Documentation of Permanency Services:
- Complete Family Assessment Service Plans (FASP), Court Reports, Permanency Hearing Reports (PHR), Termination of Parental Rights (TPR) summaries, and surrender applications as needed
- Maintain case record and other accountability records as required by agency policy and governmental regulations
- Maintain progress notes in Connections and/or Employee Health Record systems
- Update Placement Module and document movements accurately
- Writing Transfer Summaries; Incident Reports; Briefings.
QUALIFICATIONS:
- Bachelor's Degree in Social Work, Human Services, Psychology or related field required
- Experience working with families and adolescents in the child welfare system is preferred
- Must be able to establish working relationships with participants in need of supportive services
- Ability to work with diverse population of clients and team members
- Good organizational skills with ability to meet deadlines
- Must have excellent verbal and written communication skills.
- Must have a commitment to work from a strength-based, youth development and trauma informed perspective.
- Computer literate with the ability to learn new systems
- Bilingual English/Spanish preferred.
- NYS Drivers Licensed preferred to access Agency vehicles; NJ and CT license is acceptable
