Job Description
BACKGROUND:
Broadstep – Wisconsin, Inc., dba Quest Behavioral Healthcare www.qbhinc.com (Quest) has an immediate opening for a LCSW, LPC or LMFT. Under contract to the County of Milwaukee Behavioral Health Services (“BHS”), Quest operates two Crisis Stabilization Houses (“CHS Houses”), which provide services to adults with mental health, substance use, and co-occurring needs who are in need of stabilization, medication monitoring, and clinical support as an alternative to inpatient stabilization. The CSH Houses allow for continued stabilization and recovery in a least restrictive community-based setting that provides support for individuals in need. The CHS Houses are located at 5554 North 57th Street, Milwaukee, WI 53218 – “Silverlawn House”; and 1141 North 46th Street, Milwaukee, WI 53208 – “Martin House”. Each has 8-beds and is licensed as a Community-Based Residential Facility (“CBRF”) under DHS 83. The CHS Houses are governed both by DHS 83 and also DHS 34 – “Emergency Mental Health Service Programs”.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
1. Assess, diagnose, and provide treatment for clients using the DSM-5 criteria, for both substance abuse and mental health needs. The assessment shall also include an assessment of the client’s physical health and medication needs performed by a Family Practice Nurse Practitioner or Family Physician.
2. Lead and collaborate with an interdisciplinary team to develop appropriate supports and interventions, discharge planning, and service coordination. The team consists of direct care givers, peer support specialist, RN/Case Manager, Occupational Therapist, Music Therapist, and Medical Director (Psychiatrist); and other healthcare professionals as may be needed.
3. Become thoroughly knowledgeable of DHS 83 and DHS 34.
4. Become thoroughly knowledgeable of the “Crisis Stabilization House Policy and Procedure” issued by BHS, including understanding BHS requirements pertaining to the admission process, health and communicable disease screen, referrals, clinical documentation, medication monitoring, and discharge planning.
5. Ensure that clients’ co-occurring medical needs are addressed via referral and transport of the clients to healthcare professionals and facilities within the community.
6. Assist clients and families to fully utilize community resources and supports.
7. Document in Quest’s EMR system information about client’s condition, services provided, progress towards stabilization and recovery, and discharge planning.
8. Coordinate discharge planning and facilities discharge planning rounds.
9. Counsel patient/family to achieve optimal level of psychosocial functioning.
10. Demonstrate proficient skill in evidence-based practices and theories to work with adults and their families and other supports.
11. Consult with the Medical Director (Psychiatrist) as needed for advice and direction pertaining to client care as needed.

QUALIFICATIONS NEEDED FOR POSITION:
Experience and Skill Requirements: The following experience and skills are considered essential:
1. Ability to make referrals to community agencies and healthcare professionals and facilities.
2. Ability to provide ongoing communication with clients, families, physicians, and other stakeholders.
3. Can complete chart documentation.
4. Knowledge of medical conditions and impact on client care needs.
EDUCATIONAL REQUIREMENTS:
Licensed by the State of Wisconsin Department of Safety & Professional Services as LCSW, LPC, or LMFT.
