Regional Hospice is looking for Per Diem Community Visit RNs for our Home Hospice patients.
The Visit RN is responsible for follow up hospice care to the patients in our Community Hospice program. Specifically, the Community Visit Nurse will visit a patient in any setting where the patient is uncomfortable and requires symptom management, routine visits supplementing care by the case-manager who may be off or needs assistance, continuous care visits, supervisory visits of CNA/LPN, admission or evaluation visits, pronouncement visits, prn visits and family meeting visits. Patients can be in private residences, assisted living facilities, skilled nursing facilities, rehab centers, etc..
Qualifications
- Registered Nurse with a current license in the State of Connecticut REQUIRED.
- Bachelor of Science in Nursing degree REQUIRED.
- Minimum of three (3) years experience as a staff nurse in a hospital with at least one year experience in hospice/palliative care REQUIRED.
- Prior case management or home care nursing experience REQUIRED.
- Advanced credentials in hospice care preferred.
- Transportation required, including proof of auto insurance and a clear driving history.
- Excellent verbal, written and computer skills – ability to communicate effectively with patients, families, physicians, interdisciplinary team members and others.
- Fully Vaccinated for Covid-19
The Per Diem Community Visit Nurse must possess good communication and organizational skills, a professional attitude and appearance, and be able to work in a variety of environments.
Pay: $90.00 - $200.00 per day
