### Job Responsibilities:
- \*\*RAI/MDS Expertise\*\*: Possess comprehensive knowledge of the RAI/MDS process, including person-centered care planning, Medicare PDPM reimbursement, scheduling, month-end triple check, appeals process, and Medicare certification.
- \*\*Assessment Coordination\*\*: Maintain accurate PDPM/Medicare and Medicaid/Private assessment schedules, ensuring proper reference dates for distribution to therapies, dietary, social services, activities, and nursing.
- \*\*Education and Training\*\*: Provide ongoing education and training for MDS and person-centered care planning to ensure compliance and quality care.
- \*\*Regulatory Compliance\*\*: Demonstrate proficiency in RAI processes, including MDS assessments, Triggers and CAAS, and Care Planning, adhering to state, federal, and OBRA regulations.
- \*\*Medicare Certification\*\*: Manage the Medicare certification/recertification binder to meet Medicare physician certification requirements for Part A & B services.

Requirements:
*
Registered Nurse (RN) with a valid New Hampshire license
- RAC-CT certification preferred
- Minimum 2 years’ experience in long term care, including experience with the RAI process and person-centered care planning
- Must have strong clinical and communication skills
- Extensive knowledge of the RAI process and reimbursement requirements for both federal and state
- Must be highly organized with the ability to multi-task throughout the day
- Experience with Microsoft PowerPoint, Word, Excel and Outlook preferred
- Experience in developing templates to utilize in facility for audit/educational use

Pay: $80,000.00 - $85,000.00 per year
