The Barren River District Health Department serves the residents and visitors of Barren, Butler, Edmonson, Hart, Logan, Metcalfe, Simpson, and Warren counties of the Commonwealth of Kentucky. The Barren River District Health Department is accepting applications for a full-time Public Health Nurse I position for the Diabetes Program. This position will be based in Bowling Green, but will serve all eight counties (Barren, Butler, Edmonson, Hart, Logan, Metcalfe, Simpson and Warren).
Responsibilities:
- Serve as the Diabetes Population Health Specialist
- Responsible for planning and implementing Diabetes services throughout the 8 county service area
- The position will receive on the job training to provide the National Diabetes Prevention Program and Diabetes Self-Management and Support.
- This position will work to become a Licensed Diabetes Educator.
- Attend coalition meetings
- Develop relationships with community partners to facilitate referrals to programming
- Work closely with our community health worker to ensure outcomes for clients participating in classes
- Travel will be required to other counties as needed after training is complete
- Other duties as assigned.
Minimum Requirements:
- An Associate degree or diploma in nursing from an accredited college, university, or school of nursing.
- Must have RN license in Kentucky or compact state or provisional license in Kentucky, meeting all requirements of KRS 314.041
- Approval to practice as Registered Nurse Applicant (RNA).
- Applicant must have reliable transportation and a valid driver’s license.
