Site Description:
Oak Days is part of Project Homekey of Alameda County, a Permanent Supportive Housing program designed to provide individuals who have experienced homelessness. Oak Days provides permanent housing to medically vulnerable individuals who have an identified need for ongoing personal care or nursing intervention. Care is facilitated by enrollment in California benefit programs and provided by patient care teams in collaboration with Homekey clinical personnel. Oak Days aims to support residents through the care continuum and provide housing and quality healthcare with compassion and dignity.
Responsibilities:
Conduct patient/personal care as directed by nursing staff and in accordance with resident plan of care.
Support resident relationship with primary care and other professional disciplines.
Assist Registered Nurses in communication with residents including arranging and facilitating appointments with nursing staff.
Ensure adherence to infection control standards on site among staff and residents.
In collaboration with housing agency, ensure daily wellness of residents.
Participate in immunization programs at Project Homekey.
Assist in the execution of population health and safety interventions on site.
Answer calls to nursing station and respond independently to when appropriate.
Assist nursing staff with light clerical duties including filing and making copies.
Ensure that clinical documentation is current, timely, and accurate.
Maintain confidentially in accordance with the Health Insurance Portability and Accountability Act (HIPAA).
Recognize and respond to medical emergencies on site, follow up with MD communication or 911 activation.
Practice harm reduction principles on site, including interventions that minimize harmful effects of substance use while treating all residents with dignity and respect.
Other duties and responsibilities as required.
Physical Demands:
Employees may frequently need to stoop, kneel, crouch, stand, walk, sit, use arms, hands and fingers.
Employees will frequently need to communicate and listen.
Lifts Weight or Exerts Force Work Environment:
Employees may occasionally need to lift up to 100 pounds or more.
Vision: Required clear close vision, distance vision, peripheral vision, depth perception, ability to adjust focus.
Employees may occasionally be exposed to toxic or caustic chemicals.
