Come join our dynamic team! We are a small 65-bed facility with an average of 15-20 skilled rehab residents.
Responsibilities:
- Coordinate and oversee the MDS (Minimum Data Set) process for the facility
- Ensure compliance with all applicable regulations and standards related to MDS documentation and submission
- Collaborate with interdisciplinary team members to gather and assess patient information for accurate MDS completion
- Conduct thorough assessments of patients' medical, functional, and psychosocial status
- Develop individualized care plans based on assessment findings
- Monitor and update care plans as needed to reflect changes in patients' conditions
- Provide education and training to staff on MDS documentation requirements and processes
- Participate in discharge planning and coordination of post-discharge care for patients
Skills:
- Strong knowledge of MDS regulations, guidelines, and documentation requirements
- Proficiency in using electronic health record systems such as Point Click Care, or similar platforms
- Excellent communication and interpersonal skills to effectively collaborate with interdisciplinary team members
- Attention to detail and ability to maintain accurate and up-to-date records
