What will you be doing:
The Home Care Clinical Manager is responsible for mentoring, monitoring, tracking and coordinating clinical activities. In this role, you will manage the communication and coordination of quality services of the multi-disciplinary team approach to safe and exceptional patient/client care.

Provides expert guidance and assistance to the DPCS in all matters relating to the delivery of services to assure quality standards.

Demonstrates management skills needed to orient, supervise and coordinate staff, establishing appropriate accountabilities.

Performs duties necessary to maintain quality nursing standards.

Triage calls, process and coordinate doctor’s orders as appropriate; follow-up for documentation of verbal orders; ensure the timely

delivery of information received to appropriate clinicians.

Process requests for service (referrals), as needed, ensures all information is complete and accurate and forward to the appropriate personnel.

Reviews, coordinates and follows-up on 485’s, OASIS, clinical notes and DARs and ensures the documentation required for the reimbursement.
What we’re looking for in you:

Strong organizational skills, decision-making, problem solving and time management skills

Ability to be flexible and dependable and embrace change and new technology

Current professional license to practice as an RN or PT, OT, SLP in Massachusetts (in good standing)

Bachelor’s Degree required or equivalent experience
-
Two (2) plus years’ community health experience (minimum)

Two (2) years of supervisory experience preferred

Strong clinical skills

Excellent communications skills, both verbal and written

Ability to work under pressure
