Our Community Health Workers promote and advocate for all our patients. By coordinating community resources on behalf of patients and families, we are re-imagining how we deliver high-quality care to more kids in our region so they can have the best outcomes, experience, and value.
We are hiring a full-time Community Health Worker to assist with care coordination, appointment scheduling, documentation, community outreach, and in-person home/school visits. This position will work 40 hours/week, Monday-Friday on Day Shift. The scheduled hours are flexible as long as we can meet patient needs. No weekends, no holidays, no over-time, no on-call.
Qualifications: To be considered, candidates must have a high school diploma or equivalent, 2-5 years of work experience in a related job discipline, and have a reliable vehicle/valid driver's license. Current Community Health Worker (CHW) certification in the state of Ohio is preferred but not required.
Pay: $18.60-23.00/hour
Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need.

Qualifications:
EDUCATION/EXPERIENCE
Required:- High school diploma or equivalent
- 2 - 5 years of work experience in a related job discipline
Preferred:
- Current Community Health Worker (CHW) certification in the state of Ohio
