Engage all youth in transition in State run programs meeting them at least 90 days prior to discharge
· Complete the Interest Survey with all youth.
· Utilize the Interest Survey and Empower Your Future (EYF) presentation to create and maintain an individualized list of recreational and vocational interests.
· Act as a support and collaborative partner for youth in the realization and operationalization of their goals identified in their individual Empower Your Future (EYF) work.
- Identify and establish relationship with community based organizations and businesses that can provide prosocial and recreational activities within a Positive Youth Development framework.
· Catalogue these resources in one place.
· Share established resources with the DYS Casework team and other Youth Service Coordinators.
· Plan and supervise individual and group passes based on the interest of the youth.
· Assist with transportation and supervision of youth and their families for pass purposes outside of the youth’s pass menu.
· Ensure compliance with program and pass procedures.
· Participate in 90-60-30 discharge planning meetings.
· Attend monthly treatment meetings and other meetings as required/requested.
· Enter data into JJEMS, particularly targeting the Leisure and Recreation domain of the Youth Level of Service-Case Management Inventory.
- Assists in and supports the implementation of service-focused initiatives in the region as requested by DYS (i.e., Skill Up/Career, Vocational and Employability Services, DYS Youth Advisory Council).
- Supports the efforts, activities, and events of the full CSN team and DYS casework staff to ensure a team approach and to avoid duplication of services.
- Under the supervision of the Provider Contract Manager, responsible for on-site budget management, maintains a Monthly Spending Report for each tangible monetary allowance (e.g., gift card, bus passes) distributed to youth and families recording which youth/family received the monetary allowance and for what purpose
- In conjunction with the Parent Partner, produces and distributes a quarterly District Office newsletter highlighting resources, services, new initiatives, activities, information, and youth artwork to region-based staff and to the DYS Communications Coordinator for posting and archiving to the DYS Pulse (intranet).
- Assists in ensuring adequate on-site security; provides routine coverage at the District Office when needed to assist in properly supervising youth while on site – including conducting searches of youth upon entry. Maintains a youth and visitor sign-in log as needed.
Qualifications Required:
- Bachelor’s Degree in relevant field required
- Minimum of two years of experience directly relevant to job responsibilities, preferably in juvenile justice, child welfare, behavioral health, or demonstrably related field
- Microsoft Office skills
- Must be able to travel
- Must have a valid driver’s license
- Must have a personal vehicle
