This position requires you to be on site or local to commute within Chaffee County CO and Surrounding Area.
This professional will support the Women’s Wellness Connect (WWC) “Targeted Outreach Project” to provide education and scheduling support for breast and cervical cancer screening to women through the Colorado Department of Public Health and Environment (CDPHE)/Women’s Wellness Connection (WWC) Grant. This is a part-time position and is dependent on grant funding. This person will work closely with the project’s community health worker (CHW) to answer medical questions among the target population reached through the grant. The project is designed to identify and address barriers and strategies to motivate women to undergo regular cancer screening. Works remotely/off-site and in communities throughout HRRMC service region.
ESSENTIAL DUTIES PER AREA OF RESPONSIBILITY
1. Provides support to multiple target populations/backgrounds, with the goal of answering questions such as:
a. Understanding the importance and role of breast and cervical cancer screenings,
b. Verifying insurance coverage and WWC criteria
c. Advising clients on how to handle and what to expect during healthcare visits and Demonstarte screenings.
d. Answering patient questions.
2. Support the CHW in scheduling screenings and reaching annual WWC goals for each grant cycle.
3. Provide one-on-one education to clients. Use tailored communication that is focused on educating WWC-eligible women about their risks of breast and cervical cancer, the importance of regular screenings and the resources available to them.
4. Support the CHW by speaking with clients in a way that address barriers and identifies strategies for motivating women to have regular screenings.
5. Collaborate with Rural Health Clinics, County Public Health Offices, Rocky Mountain
Planned Parenthood- Salida, the HRRMC Center for Breast Health and any other agencies/organizations who are involved with and concerned about improving access to women’s healthcare.
6. Create connections between vulnerable populations and various healthcare systems and professionals in the following counties: Chaffee, western Fremont, South Park, Lake, Saguache, Custer, and in identified communities in San Luis Valley.
7. Build and maintain relationships with key communicators in each community.
8. Manage care and care transitions for vulnerable populations.
9. Advocate for underserved individuals to receive appropriate services.
10. Excellent oral, written, and interpersonal communication skills.
11. Demonstrate basic outreach skills.
12. Create a non-judgmental atmosphere in interactions with clients and their families.
13. Engage patients in ways that establish trust and rapport with them and their families.
14. Communicate effectively with patients about individual needs, concerns and assets.
15. Convey knowledge of basic health and social indicators clearly and in culturally appropriate ways.
16. Motivate patients to engage in behavior change.
17. Identify and utilize multiple community resources.
18. Actively support WWC team through meeting and event attendance, clear communication, shared expectations of assigned work, updates, and collaboration.
19. Coordinate weekly with CHW to keep each other informed/updated on plans.
20. Data entry for grant tracking
- College graduate, Associates or Bachelor’s Degree, required.
