We are seeking a highly skilled and compassionate Registered Nurse Case Manager to join our healthcare team. As a Registered Nurse Case Manager, you will be responsible for coordinating and overseeing the care of patients, ensuring they receive the highest quality of care and support throughout their healthcare journey. Duties:
- Coordinates the care plan
- Admits patients.
- Reviews case if patient admitted by another staff member.
- Familiarizes self with patient care by seeing patient at first possible opportunity.
- Assesses need for other disciplines/DME as appropriate,
- Obtains orders as needed to initiate additional services.
- Implements and follows up with staff / management for identified care issues.
- Ensures appropriate utilization of services.
- Re-evaluates and /or updates POC, as needed.
- Ensures appropriate recertification completed within mandated timelines.
- Communicates / coordinates care with other staff members and disciplines, MD, patient and caregivers, as appropriate. Documents these communications.
- Obtains ongoing MD orders and collaborates with MD in fulfilling patient care needs.
- Reports status changes to patient’s MD and other disciplines, as needed.
- Provides current visit instruction for continuity of care, as needed.
- Assures development and updates to HHA care plan.
- Assures HHA supervision is conducted according to regulatory and agency policy requirements.
- Is responsible for preauthorization of managed care patients
- Coordinates patient care conferences as appropriate and assures coordination of care is documented.
- Notifies team with transfer of case management responsibilities whether temporary (i.e., vacations), or permanent (i.e., discipline change).
- Delegates responsibilities, as appropriate
- Plans for discharge: (Begins at admission and is an ongoing process; is based on Medicare/payor criteria and Agency criteria for discharge).
- Coordinates and communicates plans to discharge with patient and team members.
- Provides verbal and/or written instruction regarding patient’s ongoing needs following discharge.
- Completes discharge procedures per Agency protocol.
- Assists in development of home health protocols, procedures and policies as requested.
- Maintains and updates level of knowledge and skills required to provide quality home health patient care.
- Participates in QA/PI activities and in-service education related to areas of practice as appropriate.
Qualifications:
- Current California RN License.
- Graduate of an NLN accredited nursing school.
- Meets competencies per Agency guidelines.
- Minimum one year of professional Home Health or Acute Care nursing experience in the last three years.
- California driver’s license and automobile insurance with minimum Bodily Injury Liability coverage per agency policy.
- Current CPR certificate.
Shift:
M-F 8 hour day shift
Job Type: Full-time
Pay: $120,000.00 - $145,000.00 per year
