JOB SUMMARY:
Under the supervision of the HRM Program Manager the incumbent is responsible for the day-to- day provision of harm reduction mental services which consist of multiple concrete services within a psychosocial context, to include referrals, advocacy and crisis intervention. All responsibilities must follow Public Health Solutions, NYC DOHMH, BAC and AIDS Institute guidelines for HIV Positive Individuals.
Education:
- Minimum: High School Diploma or equivalent
- CASAC-T/ CASAC.
- BA degree from an accredited college or university.
Work Experiences:
- At least 3 years’ experience with Substance Abuse and HIV/AIDS issues, including engagement and motivation, and referral for testing and health care services.
- Good verbal and writing skills.
- Flexibility a must.
- Familiar with using the EHR system.
Bilingual: A plus but not required.

DUTIES AND RESPONSIBILITIES:
- Serve as the primary Harm Reduction Counselor to HIV+ clients with a history of or present substance use.
- Conduct outreach and in-reach to clients and engage them in comprehensive risk assessment to determine a match for HRM services.
- Maintain up to date and accurate individual records.
- Work in a team model and ensure on-going contact with clients on caseload.
- Complete assessments, reassessments and service plans as specified by BAC and Public Health Services in a timely manner.
- Record all contacts and transactions with clients, collaterals, and service providers in the eCR.
- Conduct weekly group and individual sessions with clients.
- Meet contract deliverables per contract mandates.
- Develop a comprehensive service plan, including coordination of services and provider referrals using the SMART Goals method.
- Provide follow up for all client referrals, missed appointments, and missed individual/group sessions
- Complete client closing and transfer summaries with approval from Program Manager.
- Provide supportive counseling to assist with implementation of service plans and encourage client self-sufficiency and empowerment.
- Monitor counseling services to ensure that services are provided, and client is engaged in care plan.
- Promote compliance with medical and social support appointments
- Participate in case conferences (both internally and externally) as needed.
- Submit daily logs, depicting work completed with clients.
- Take part in assigned agency committee meetings.
- Develop individualized treatment plans based on clients' goals and needs
- Provide individual and group counseling sessions using evidence-based therapeutic approaches
- Offer crisis intervention services to clients in acute distress or experiencing a mental health crisis
- Other duties as assigned, based on program needs.

- Knowledge of community resources for people with chronic medical and behavioral health issues.
- Second language (Spanish, French and or Creole) a plus but not required
- Knowledge of HIV/PrEP and PEP
- Knowledge of diagnostic evaluation techniques and ability to apply ICD-10 codes accurately
