individuals and families in the City and County of Alameda through several programs: Early
Head Start, Head Start, Family Support Services, School Based Services, and Behavioral Health

SCHOOL BASED SERVICES
Alameda Family Services’ School Based Services Program provides a multitude of health and
wellness services to youth on school campuses in the Alameda Unified School District. Services
include individual and group counseling, case management, family engagement, as well as health
education, and youth development.
OVERALL RESPONSIBILITIES:
A Clinic Coordinator works under the direct supervision of the Health Centers Manager. The
Clinic Coordinator is responsible for providing administrative and clerical support to the School-
Based Health Center (SBHC) through coordinating daily operations and services. This position is
responsible for reception, scheduling, health education, billing, & data entry, as well as
supporting program-wide mental health outreach and youth development.
The Clinic Coordinator position focused on health education and outreach, based out of our
health & wellness center is responsible for coordinating health education, youth development,
and health and wellness services for the continuation high school in the Alameda Unified School
District. This includes being responsible for developing, scheduling, and facilitating classroom
presentations, coordinating health and wellness services, as well as utilizing research-based
curriculum to facilitate youth development, peer education, and leadership training.

● Establish, maintain, and coordinate services for our Island Health & Wellness
Center.
● Coordinate and conduct outreach, awareness campaigns, and educational
presentations on a variety of health topics to students, parents, and community
groups.
● Development of health education and outreach materials such as curriculum,
workshops, pamphlets and bulletins.
● Provide health education services individually and in small-groups.
● Coordinate health and wellness services offered through Alameda Family Services
and Native American Health Center.
● Create youth friendly health education, outreach, and marketing materials.
● Maintain all health education materials and resources including brochures,
pamphlets, magazines, posters, and videos.
● Accurately document health education and outreach efforts as needed for program
evaluation and contract compliance.
● Collaborate with Health Centers Manager to recruit and support high school peer
health education and leadership group and college health education interns.
● Act as program representative on campus and in the community.
● Collaborate with school administration, faculty and staff to support with school
initiatives including family engagement and student attendance improvement.
● Attend and participate in a variety of program and agency meetings, as well as
professional development trainings.
● Support with School-Based Health Center and clinic services as needed.
● Other duties as assigned by Program Director & Health Centers Manager.

QUALIFICATIONS, SKILLS AND EXPERIENCE:
● Bachelor’s Degree
● 1 year of both administrative assistance and clerical experience OR 2 years of secretary,
reception, or clerical experience.
● Professional health education and community engagement experience.
● Experience working with multicultural populations.
● Knowledge and experience in youth development practices preferred.
● Energy, enthusiasm, and a desire to work with adolescents in a health care setting.
● Knowledge of procedures and protocols of working within a school setting.
● Skills in organization and record keeping.
● Google Drive and Microsoft Office Suite proficiency
● Knowledge of Canva and/or other design and publishing tools
● Experience with electronic health records (EHR) systems preferred
● Ability to appropriately interact and support students, parents, co-workers, and school
faculty.
● Ability to prioritize and manage multiple responsibilities.

PHYSICAL JOB DESCRIPTION:
● Must be able to sit for at least 85% of the time and stand for 15%.
● Must be able to lift up to 30 pounds (books, files, etc.).
● Must possess valid California driver’s license & have reliable transportation to travel
between sites.
CONDITION OF EMPLOYMENT:
1. The School Based Services program requires a background check and clearance from
the Federal Bureau of Investigation (FBI) and Department of Justice (DOJ)
background prior to employment.
2. A current TB Test Results is required prior to employment.
3. Proof of COVID Vaccination and Booster.
