Case Manager - Bachelor’s Level
The Fall River Deaconess Home is a private, non-profit, community-based agency serving youth and their families with a comprehensive continuum of residential, educational, clinical, medical, and supportive services. Our agency provides services in a trauma-informed environment designed to create a therapeutic environment to enrich the lives of youth from Massachusetts, Rhode Island, and Vermont.
Job Profile
The Case Manager reports directly to the assigned Clinical Director and is a member of the clinical and milieu team providing therapeutic, life, vocational, and social services to the youth in our residential program and their perspective families. Some evenings and weekends are required.
Schedule
Sunday - Thursday with a rotating on-call requirement and alternating holidays
- Sunday: 11am-7pm
- Monday: 11am-7pm
- Tuesday: 9:30am-5pm
- Wednesday: 9am-5pm
- Thursday: 11am-7pm
The successful candidate will:
· Co-facilitate specialized clinical groups with clinicians
· Implement individualized treatment and behavioral support plans
· Complete CANS assessments, youth readiness assessments and progress notes
· Provide case coordination for youth and families
· Act as a liaison between the program, schools, state agencies, places of employment, and families
· Participate in on-call responsibilities on a rotating basis
· Assist in day-to-day milieu responsibilities
· Identify available community-based resources and manage referrals, engagement, follow-up, and coordination of services
· Attend and participate in supervision, staff meetings, trainings, conferences, and workshops to promote professional development
· Work with youth and families to accomplish goals within a congregate care setting, their home, and the community
· Creating and implementation of summer day program for Group Home youth
Qualifications
· Bachelor’s level degree in Social Work or related field (required)
· Experience in social service field (preferred)
· Positive disposition and a team-player mentality
· Excellent communication skills
· Well-developed writing skills
· Flexible approach to case management
· Strong organizational skills
