Henry Mayo Newhall Hospital (HMNH) is a 357 bed acute care, independent non-profit hospital with a Level II Trauma Center and supports 71 specialties and sub-specialties.
HMNH is known to receive prestigious accreditations and recognition in a multitude of specialty designations for excellence in healthcare, the hospital offers an array of services, including newly opened fitness and health center, maternity department, cancer program, advanced primary stroke center and spine and joint program. The hospital is an Advanced Primary Stroke Center and a STEMI Receiving Center, as well as part of the Los Angeles County Disaster Resource Center (DRC) network.
We are in search of a Case Manager, RN to fulfill a full-time day shift (8:30AM-5:00PM) position in our Care Management department.
RNs hired into eligible position can receive a Sign-On Bonus of up to $5,000!
As a Case Manager, RN you will:
- Be responsible for coordination of care, utilization review, acting as a resource to patient, family and nursing units; referrals to clinical social worker for psycho-social issues and complicated discharge planning issues; and overseeing coordination of post-hospital care needs; interacting with insurance companies, medical groups, and community resources to ensure timely, efficient use of all services.
- Apply knowledge of the expected clinical process, available resources, medical necessity criteria, regulatory requirements, and relevant standards of care in anticipating patient needs.
- Be responsible for case management across the continuum of care utilizing an interdisciplinary approach to managing patient throughput in a fiscally responsible manner to achieve optimal outcomes.
- Work as a member of a multidisciplinary team to actively facilitate those functions associated with the patient throughout an acute episode of care.
We are looking for a Case Manager, RN who has:
- Current CA RN license
- Current BLS provider card card from American Heart Association
- Case Management certification preferred
- Diploma, AA, BS in Nursing
- MS in Nursing preferred
- Two (2) years as a Case Manager in an acute care setting or five years recent acute care nursing experience preferred.
- Strong interpersonal skills, leadership ability, negotiation skills, and knowledge of hospital operations.
- General knowledge of the payer industry, resource management, reimbursement, and evidence-based clinical practice is essential.
- Knowledge of InterQual and/or Milliman preferred

Job Type: Full-time
Pay: $47.90 - $70.74 per hour
