The Wound Care Nurse functions under the general supervision of the Clinic Manager and the clinic physician. The Wound Care Nurse is responsible for assisting in the management and treatment of patients with chronic, non-healing wounds, ostomies, abdominal stomas, ulcers and other skin conditions.
Reports to: Clinic Manager
Essential Duties:
- Accurately performs and documents patient assessments and identifies patient needs.
- Evaluates progress and outcomes.
- Assesses, determines classification, stages, measures, and photographs wounds accurately and in accordance with policy.
- Identifies and initiates appropriate nursing interventions.
- Documents care plans and progress toward goals.
- Provides patient and family teaching as it pertains to the treatment plan, procedures, and preventative measures throughout the course of treatment.
- Provides direct care appropriate to condition and age of the patient.
- Works cooperatively and actively participates with other members of the care team. Assists other team members to help balance the workload.
- Participates in continuous quality and performance improvement. Communicates observations and suggestions related to opportunities for improvement.
- May provide input into the budgetary planning process and contributes to the cost effectiveness of services, products, and programs provided by the center.
- Maintains positive relationships with all customers. Maintains a positive attitude and remains patient focused.
- Complies with regulatory and accreditation standards as well as Center policies and procedures.
- Responsible for personal and professional development.
- Maintains ethical practices and HIPAA requirements.
- Performs timely and appropriate documentation in the Center’s EMR.
- Provides a safe environment for patients, visitors and staff and complies with appropriate and approved safety standards.
- Collaborates as needed with other healthcare providers, referral sources, Center Manager, Safety Director, and the Medical Director regarding clinic and patient needs.
- Assists in inventory management by identifying supply needs in advance of depletion. Notifies clinic manager of clinic needs.
- Performs case management as needed to ensure physician orders and referrals are carried out. Schedules outside referral appointments as necessary to ensure continuity of care.
- Ensures ordered test results and/or referrals are received prior to next clinic visit. Communicates significant findings as appropriate to the physician.
- Assists in maintaining a clean and organized clinic environment.
- Assists in staffing other clinics as needed for the business needs of the organization.
- Other reasonable duties as assigned.
Qualifications:
- Current unencumbered RN or LVN licensure in the state.
- Current Basic Cardiac Life Support, required.
- At least 2 years of current experience in wound care, preferred.
- Wound Ostomy Care Nurse (WOCN), preferred.
- Training and experience in wounds Vac, preferred.
- Advanced Cardiac Life Support, preferred.
- Due to nature of business and role, Covid-19 vaccination or an approved exemption is required.
