We are searching for a full-time outpatient case manager to join our team in the Towanda office. The ideal candidate for this exciting career will have integrity and a passion for helping others. Working closely with the clinical supervisor, you will be processing intakes, discharges and scheduling requested evaluations. With this position, you will need to be very organized and efficient with all paperwork.
What Do I Need?
Education and Experience
Bachelor's Degree required in human services or related field
AND
Two years of experience in the field of human services.
Other Requirements
- You will maintain a valid driver's license and have the ability to travel.
- You will maintain all required clearances.
- You will have the ability to use typical office equipment.
- You will have the ability to work as a part of a team and also independently.
- You will have excellent organizational, interpersonal, and communication skills.
- You will follow all HIPPA privacy regulations.
What Will I Do?
- You will work closely with the clinical supervisor regarding caseload activity, admissions and discharges.
- You will process incoming clients for the appropriately assigned BHS program.
- You will be in contact with the clients and families and therapists to ensure everyone is up to date with information.
- You will schedule psychological evaluations after obtaining authorization.
- You will facilitate interagency meetings and handle corresponding paperwork.
- You will prepare and maintain your client files and participate in all meetings within the agency pertaining to your client.
- You will work in a professional manner and cooperate with CONCERN staff and other agencies.
- You will take responsibility for all formal reports and contacts with all internal and external people involved.
Hours of Work
Varied hours dependent upon needs Monday - Friday.
