Job Overview
Under the general direction of the Jordan Valley Senior Care Medical Director, develops and implements the quality improvement program for Jordan Valley Senior Care. This includes annual review and update of the quality improvement plan, development and implementation of program wide and center-specific quality indicators, data management, education and training of program staff around quality improvement, preparation of routine and special reports, and coordination of improvement efforts arising from monitoring activities.
Duties:

Conducts an annual review of the quality improvement program for Jordan Valley Senior Care and updates plan as appropriate. Prepares routine and special reports of quality improvement activities and performance.

Coordinates the development and implementation of program-wide and center-specific quality indicators.

Oversees data management for QI activities including the coordination of collection, entry, analysis and assessment of data.

Plans and oversees education for Jordan Valley Senior Care staff on QI activities and techniques.

Assists in interface between consumers and program management for quality-related complaints.

Attends and supports the Quality Improvement Committee of the Jordan Valley Senior Care Board of Directors on Jordan Valley Senior Care-related matters.

Coordinates the internal and external appeal process.

Performs root cause analysis process as applicable.

Responsible for compliance with CMS PACE and State PACE quality data, monitoring, and reporting.

Other projects and duties as assigned.
Requirements:
Required Qualifications

Minimum two years experience working in healthcare quality improvement.

Current valid RN license
-
BLS Certification required within 90 days of hire.
Preferred Qualifications

Bachelor’s or Master’s degree in health care administration or related field
