Overview:
We are seeking a dedicated Community Case Manager to join our team. The ideal candidate will have experience in workforce development, motivational interviewing, crisis management, Section 8, behavioral health, addiction counseling, and social work.
This is an on-site position. Clients are on-site for case management services.
4-day-per-week position; Office hours 8:30 am - 5 pm
Duties:
- Conduct comprehensive assessments to determine clients' needs and develop individualized service plans
- Provide guidance and support to clients in accessing community resources and services
- Collaborate with interdisciplinary teams to ensure coordinated care for clients
- Monitor and evaluate clients' progress towards goals
- Advocate for clients' rights and needs within the community
- Maintain accurate and up-to-date case notes and documentation
Requirements:
- Bachelor's degree in social work, psychology, counseling, or related field
- Proven experience working with diverse populations in a social services setting
- Strong communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Knowledge of local resources and services for individuals in need
- Certification or licensure in relevant areas is a plus
