Registered Nurse Case Manager
Job Summary:
Join our healthcare team as a Registered Nurse Case Manager, where you will play a crucial role in coordinating patient care and ensuring optimal outcomes. We are looking for a dedicated individual with experience in Pediatrics, Primary care, Outpatient, Geriatrics, Infectious disease care, Inpatient settings, and Hospital environments.
Duties:
- Provide comprehensive patient care management by assessing, planning, implementing, and evaluating individual patient needs.
- Collaborate with healthcare professionals to develop and implement care plans tailored to each patient's unique requirements.
- Conduct diagnostic evaluations and provide acute care interventions as needed.
- Coordinate with interdisciplinary teams to ensure seamless transitions of care for patients across different settings.
- Advocate for patients to ensure they receive high-quality and timely healthcare services.
- Document all patient interactions and interventions accurately in accordance with regulatory standards.
Skills:
- Proficient in patient care management and coordination.
- Strong diagnostic evaluation skills.
- Ability to work effectively in fast-paced and high-pressure environments.
- Excellent communication and interpersonal skills.
- Knowledge of acute care practices and procedures.
- Familiarity with electronic health records systems.
Requirements:
- Active Registered Nurse license.
- Bachelor's degree in Nursing preferred.
- Certification in Case Management a plus.
- Minimum of 2 years of experience in nursing, preferably in case management or related field.
