Summary
The Population Health Director is responsible for managing all population health initiatives and assisting with the transition of Crossroad Health Center to a value-based model of care.
Responsibilities
· With the assistance of the Quality Manager, supervise and oversee all population health management activities for all Crossroad sites, including Comprehensive Primary Care
· Improve the health outcomes of a group by monitoring and identifying individual patients within that group
· Administer care management programs
· Identify and address care gaps within the patient population, especially in communities with chronic illnesses and diseases
· Coordinate data to track the results of care
· Use IT solutions to understand outcomes from the data that is collected
· Manage population health team, including care managers, care coordinators, community health workers, family resource coordinators and other team members as assigned
· Along with the assistance of the Human Resources team, recruit, interview and hire new population health team members as needed for each site
· Coordinate initial and ongoing training to population health team
· Promote efficient, organized and pleasant office operations in the health center, consistent with the mission of CHC to offer care in an atmosphere of dignity and respect
· Uphold all practices for the Health Insurance Portability and Accountability Act (HIPAA)
· Serve as a representative of the health center to the community by communicating in a friendly, caring manner both on the phone and in person
· Participate in and be supportive of the completion of all Patient-Centered Medical Home efforts and certification
Knowledge, Experience and Skills
· Registered Nurse
· Clinical proficiency-the ability to identify and address care gaps within the patient population, especially in communities with chronic illnesses and diseases
· Demonstrated management skills
· Direct care management experience in a primary care setting
· Experience in data collection and interpretation of data, know computer technology, and have a keen understanding of the healthcare industry.
· Able to work independently, yet seek resources or assistance when needed
· Able to handle a variety of responsibilities, initiate projects, delegate/supervise work
· Good organizational skills; Able to coordinate multiple tasks well
· Good communication skills, verbal and written
· Ability to work with staff in a Christian environment and encourage the faith-based aspect of Crossroad’s mission
Job Type: Full-time
Pay: $65,000.00 - $75,000.00 per year
