Hinds Hospice, one of the most well-known and well-respected nonprofit agencies in California’s Central Valley and a pioneer in the practice of hospice and palliative care, is seeking a RN, Patient Access Specialist to join our team.
Reporting to the Director of Patient Access and After Hours, the Patient Access Specialist, RN's responsibility is to perform pre-admission evaluations to determine appropriateness for hospice services, initiate the admission process by educating and informing patients and families of the hospice benefits and services, and completes the hospice eligibility assessment, coordinates attending physician services, and completes start of care paperwork, including the election of benefit.

Coordinate any services outside of the scope of practice of the LVN Intake Coordinators

Provide highly effective, customer service to customers and consumers

Qualifications include but are not limited to:

Registered Nurse License from an accredited school

Bachelor's Degree preferred
-
Two (2+) years of experience in hospice

Experience working independently

Excellent verbal, written & interpersonal communication skills required.

Possess reliable transportation, a valid CA driver's license, proof of auto insurance, and current auto registration.
