ESSENTIAL FUNCTIONS:
· Provides coverage on the HIV testing phone and conducts daily outreach
· Conducts HIV prevention intervention specifically Community Promise, ARTAS, and POL.
· Conducts HIV Counseling and Testing utilizing Alere Ag/Ab combo HIV Rapid Test
· Recruits participants for interventions and role models for Community Promise, PrEP and PEP.
· Completes required documentation
· Participates in internal and external trainings and skill enhancement seminars
· Produces timely monthly and quarterly program data reports
· Participates in staff team meetings and other meetings as required
· Work collaboratively with the Youth, and Men’s, programs to ensure that all patients are screened for HIV, HCV, and STIs.
· Participates the implementation of two DEBIs – Community promise and RESPECT
· Participates comprehensive HIV Counseling and Testing program•
· Ensure that all data collected and entered into the e2Education electronic record system.
· Provide information and referrals services to patients.
· Provide individual risk assessment to Youth determined to be at high risk of acquiring HIV and other related sexually transmitted diseases (STDs); perform HIV preventing counseling as needed to support the HIV integration process within WSHC and provide HIV education to clients.
· Conduct sessions on HIV, hepatitis and STI education, using age appropriate, culturally sensitive, agency approved materials/models and approaches to clients accessing services or staff providing services.
· A demonstrated understanding of up-to-date youth risk factors, and the use of technology to conduct outreach.
· Help youth to identify personal risk factors and barriers to risk reduction and work with youth to develop and achievable personalized risk-reduction plan, support youth-initiated behavior change and help youth develop a longer-term risk-reduction plan.
· Conducts risk assessments and make appropriate client referrals and/or follow-up based on HIV test/STI screening results and other presenting problems; perform partner elicitation and make appropriate case management referrals for clients with positive HIV test results.
· Perform outreach activities within the community to promote HIV, hepatitis and STI education, emphasizing HIV integration services occurring within the health center.
· Review client records weekly and ensure that documentation is properly maintained by clinic and support staff according to established department and programmatic policies including confidentiality of medical records.
· Records and submit required reports and documents in timely manner.
· Maintain current information on HIV/AIDS, hepatitis, STDs, and local resources.
· Adhere to local, state and federal regulations and policies that govern provision of HIV services.
· Performs other duties as requested.
Required Experience/Abilities/Competencies:
· Minimum one-year experience in conducting outreach and/or delivering evidence-based HIV prevention interventions
· Experience with substance abuse issues and treatment resources
· Knowledge of HIV infection and related services
· Knowledge of the targeted communities, their needs and how to effectively reach them
· Ability to conduct HIV Counseling and Testing utilizing Alere Ag/Ab combo HIV Rapid Test
· Ability to conduct Community Promise (HIV prevention intervention)
· Ability to communicate effectively in writing and verbal communications
· Ability to work evenings and weekends
· Ability to be linguistically and culturally competent
· Proficiency in Microsoft Office Suite and Outlook

Pay: $23.00 - $24.62 per hour
