Community Liaison/Hospital Screener
Baptist Health Nursing and Rehabilitation Center (BHNRC), one of the Capital Region’s premier nursing homes, is seeking a Community Liaison to join our team!
The ideal candidate will have prior experience in a similar setting, preferably a healthcare organization.
This position is responsible for reviewing and completing assessments on referrals made to Baptist Health Nursing and Rehabilitation Center for both long term care and rehabilitation stays.
Job Requirements:
Education– This position requires at least one year of experience in a geriatric medical field, with a degree in one of the following: Nursing, LPN/RN, Social Work, or related field. Valid NY Driver’s License. Must be able to obtain credentialing at referral hospitals in order to screen potential admissions.
Experience– Two or more years of experience working as a hospital screener or in a residential facility with a geriatric population is desired.
Skills– Ability to set priorities and work independently as well as to accept and utilize constructive feedback to enhance professional skills. General knowledge of the geriatric population and an understanding of basic medical terminology and related insurances, Medicaid and Medicare.
·Essential Functions of the Job:
1. Assess and evaluate patients in area hospitals and the community for admission to both sub-acute rehab and long term care beds and make bed offers as required.
2. Conducts assessments and analyze medical records in order to ascertain appropriateness of the patient to transfer to a SNF.
3. Maintain constant communication with hospital case management staff to facilitate transfers to SNF.
4. Maintain communication with insurance companies to ensure coverage and obtain prior authorizations for insurances that require such.
5. Conduct marketing to local hospitals, assisted living and adult homes in the area to maintain positive relationships for future admissions.
6. Maintain a log of screens completed in both the community and the hospitals. Produce reports for Admissions Coordinator as requested.
7. Complete medical bed hold forms, have signed by hospital coordinator.
8. Facilitate the admissions process of new residents, ensuring that they and their responsible parties are aware of their rights and responsibilities as residents of the nursing home.
9. Remain current with advances in professional practice and any license or certification requirements.
10. Participate in supervision meetings and training as required.
11. Assist with tours for potential residents/patients.
12. Explain and assist resident/patients with insurances, Medicare, and Medicaid applications/process.
13. Assess drug costs for potential admissions.
14. Travel to area referral sources (i.e., hospitals, nursing homes, home evals, etc.)
