GENERAL SUMMARY:
Sign-On Bonus! Up to $5000.
Under general supervision, the incumbent provides psychiatric and substance abuse services to patients throughout the Hospital. They assess clients' psychosocial needs and issues, develop treatment goals/plans, and offer clinical intervention to individuals and families to facilitate treatment and recovery.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
- Obtain psychosocial history from patients and/or family/significant others.
- Conduct supportive psychotherapy with individuals, groups, and families.
- Develop individualized treatment goals and plans based on psychosocial assessment.
- Participate in multidisciplinary team meetings to develop and review treatment plans and patient progress.
- Create and implement patient discharge plans to support optimal psychological and social functioning upon returning to the community.
- Collaborate with courts, Department of Social Services, Community Mental Health, private psychiatrists, family agencies, and other community resources for post-discharge follow-up care.
- Document information in patients' charts regarding Social Work & Assessment status and interventions.
- Provide consultation services for patients requiring substance abuse counseling.
- Offer crisis intervention counseling and educational groups for patients needing substance abuse and/or psychiatric services.
- Lead or co-lead psychotherapy groups or classes when applicable.
- Coordinate patient discharges from the Psychiatric Unit, including arranging appropriate support for follow-up.
- Attend and provide in-service education as requested.
SUPERVISION:
Reports to the Director of Social Work.
KNOWLEDGE, SKILLS, AND ABILITIES:
- MINIMUM EDUCATION: Master's Degree in Social Work and current State of Michigan License (LMSW or LLMSW) as a Clinical, Medical, or Psychiatric Social Worker required.
- MINIMUM WORK EXPERIENCE: Minimum one (1) year of Case Management experience required. Two (2) years current postgraduate experience in a psychiatric setting preferred. Current hospital-based experience preferred.
- CONTACT WITH OTHERS: Interpersonal communication skills required to effectively interact with all members of the health care team, family members, and the community. Expected to float or be pulled to any other Behavioral Health area as needed.
- ANALYTICAL DEMANDS: BLS (annual requirement), Recipient Rights Training, CPI-Nonviolent Crisis Intervention, TB Skin test, and mask fit (immediate upon hire and annually). Fingerprinting (immediate upon hire). Neat, clean, and professional appearance.
- CERTIFICATION, LICENSURE: Current State of Michigan License (LMSW or LLMSW) as a Clinical, Medical, or Psychiatric Social Worker required.
WORKING CONDITIONS:
- PHYSICAL EFFORT: Long periods of sitting, standing, and walking. Must be physically able to manage patients per CPI-Non-Violent Crisis Intervention approved techniques, using the least restrictive intervention.
- WORK ENVIRONMENT: Exposure to potentially violent and emotionally volatile patients.
- EQUIPMENT USED: Equipment necessary to perform specific tasks/duties.
