Essential Functions and Responsibilities: Under the supervision of the Housing and Wellness Director , the Care Manager is responsible for, but not limited to:
Client Services
- Conduct outreach, intake, and needs assessments with potential program participants.
- Provide non-judgmental guidance for program participants while promoting individual growth, independence and increasing social responsibility utilizing evidence-based practices such as motivational interviewing.
- Create and update personalized care plans with participants.
- Interview and provide screening of potential residents, while administering intake and discharge procedures.
- Connecting clients with social service programs and aiding with State and County benefit establishment, obtaining documents, employment linkage and assistance, assistance with navigation of housing applications, referrals and linkage to health care services, substance abuse services, and referral to other community support as necessary.
- Assisting clients with transportation assistance and translation services.
- Act as liaison for clients with healthcare providers to facilitate client treatments.
- Provide recurring follow ups with clients to evaluate individual progress and assist with continued care to ensure an improved health outcome.
- Coordinate appointment scheduling, prevention and screening recalls, monitoring referrals, and responding to telephone messages.
- Maintaining accurate and detailed records of case management activities.
- Facilitate crisis intervention, prevention, and crisis management on an as needed basis.
- Assisted in expanding services into rural Sonoma County, including outlying, and underserved rural areas of West County and Healdsburg, focusing on greater inclusion of Latinx and Tribal Nation populations and hard-to-reach marginalized individuals.
- Utilize innovative outreach strategies to reach these underserved populations.
- Aid in increasing coordination among Sonoma County healthcare partners focusing on Tribal health, managed care, and local area hospital partners.
- Assist in providing ECM services to Transitional Aged Youth experiencing homelessness.
Program Requirements
- Participate in scheduled individual supervision, staff meetings, in-service training and other meetings as required.
- Participate in program planning and development.
- Comply with all confidentiality guidelines and respect staff/client boundaries.
- Adhere to agency policies and procedures.
- Maintain a positive and respectful attitude with all work-related contacts.
- Consistently reports to work, on-time, prepared to perform the duties of the position
- Assist in increasing equity in our Sonoma County community by helping our most marginalized community members meet their basic needs and achieve improved health.
Recordkeeping
- Document all work with program participants in a timely manner through appropriate record keeping in compliance with program policies and procedures.
- Ensure all interactions with clients are accurately documented in the ECM database within hours of interaction.
- Report on all special incidents verbally and in writing to the Director of Housing and Wellness.
Other duties as assigned.
Minimum Qualifications:
- BA in Psychology, Social Work, Rehabilitation, Counseling, or a related field preferred
- *May substitute with a High School Diploma and 2 years case management or related field experience.
- Must have a Valid CA driver’s license, personal Auto insurance and ability to be insured by CSN’s insurance agency;
- 2+ years of recent case management experience.
- Demonstrated ability to relate to clients in an actively engaging and empathetic manner.
- Excellent interpersonal and coaching skills; strong communications and proven ability to interact with Clients and provide excellent documentation.
- Must have demonstrated ability to work with a diverse and multicultural population.
- Proven ability to work both independently and on a team
- Working knowledge of Word, Excel and other standard computer programs
- Ability to perform client outreach in the field 75% of the work day.
- Ability to manage and effectively service a caseload of 20+ individuals per month.
