JOB SUMMARY:
Responsible for performing nursing and care management services under the direction of physicians, nurse practitioners and/or the Population Health Dept Manager. The CCC May be assigned to any AccessHealth site.
ESSENTIAL JOB DUTIES AND RESPONSIBILITIES:
1. Performs care management/nursing services within the scope of the assigned clinic, under the direction of the providers and/or the Population Health Dept Manager
2. Responds to patient telephone inquiries.
3. Provides appropriate patient and family education about illness, needed treatment, return visits, medications, and immunizations of an assigned group of patients.
4. Assists the provider in the management of chronic conditions of an assigned group of patients.
5. Performs home visits as deemed necessary by management.
6. Performs emergency procedures according to approved protocols.
7. Responds to refill requests from assigned group of patients.
8. Coordinates with other AccessHealth departments, and outside entities within the county, and state to provide a cohesive program of service for agency clients and the community.
9. Sets, tracks, monitors, and assists patients with reaching their health goals by utilizing a holistic approach to nursing.
10. Accepts job related training and development in new tasks and technologies.
11. Comply with all company policies, procedures, and regulatory requirements.
12. Performs all other duties as assigned by management.

While performing the duties of this job, the employee is not exposed to weather conditions. The noise level in the work environment is usually moderate.
Hours:
Work hours are generally a regular work day of 8:00 a.m. to 5:00 p.m. five days per week, Monday through Friday. However, the position may require work in addition to regular hours, before 8:00 a.m., after 5:00 p.m., and occasional weekends.
EDUCATION/LICENSURE/CERTIFICATION QUALIFICATIONS:
REQUIRED:
1. Graduate of an accredited vocational nursing program.
2. A valid Texas RN or LVN license. License must be kept current to retain employment.
SKILLS AND EXPERIENCE:
REQUIRED:
1. Two years’ work experience in a clinical environment
2. Well–organized and self-directed.
3. Ability to relate to people at all levels in the organization.
4. Excellent communication skills.
5. Ability to work with people from all socioeconomic levels.
6. Ability to respond effectively to the most sensitive inquiries or complaints.
7. Consumer advocate for quality health care and services.
8. Ability to handle multiple projects.
9. Ability to read, analyze and interpret documents.
10. Ability to operate computer, copier, and fax machine.
PREFERRED:
1. Bilingual ability preferred.
2. Case/care management experience preferred.
