Qualifications:
Registered Nurse with at least 2 - 3 years experience in Home Health a must. Bilingual preferred, though not required.
Responsibilities:
- Coordinates and manages all services rendered to patients and/or families.
- Participates in the assessment, evaluation and planning of patient care, and revising as necessary to include completion of all documentation.
- Administers nursing care as ordered by the nursing care plan and MD orders.
- Completes the necessary forms and obtain doctor’s order and authorization for the service; utilizes the appropriate charting format and documents timely and accurately, assuring that documentation on chart correlates with MD orders.
- Communicates with the regional nurses, caretakers, and MD regarding clients’ problems; identifies and communicates significant abnormalities in the patients’ status to appropriate personnel and coordinating healthcare providers.
- Orient field staff to his/her duties and responsibilities.
- Interfaces with patients, insurance, physicians and referral sources as needed.
- Assures that change of orders are completed as needed; assures that all disciplines are scheduled based on plan of care, ordered frequencies and that visits are made timely.
- Participates in the auditing and revision of Agency policies and procedures affecting clinical service.
- Inform and confer with administrator on all patients and personnel needs and problems.
- Serves as patient advocate, assessing and assuring that quality care is given.
- Acts as Alternate Supervising Nurse in the absence of the Supervising Nurse.
- Other duties as assigned by the Supervising Nurse.
