South Valley is the newest nursing facility in Denver and we are seeking top-notch individuals. Essential Duties and Responsibilities:
Conducts and coordinates the development and completion of preliminary and comprehensive resident assessment (MDS) in accordance with current rules, regulations, and guidelines.
Ensures that the care plan includes measurable objectives and timetables to meet the resident's medical, nursing, mental and psychosocial needs, that appropriate health professionals are involved in the assessment, and that the schedule of activities is developed as required.
Conducts or coordinates the interviewing of each resident for the assessment.
Helps develop and update a comprehensive resident assessment and care plan for each resident. Ensures that it is updated on a quarterly and annual basis and upon significant change in condition.
Evaluates each resident's condition and pertinent medical data to determine any need to amend the admission assessment.
Qualifications:
LPN license in accordance with CO State law
- Previous experience in nursing, patient care, or other related fields
- Ability to build rapport with patients
- Compassionate and caring demeanor
- Familiarity with medical terminology
- Ability to work well in teams
