Job Summary:
The ECM Leader plays a pivotal role in guiding and supporting a team of Lead Care Managers (LCMs), ensuring the provision of high-quality care to members. Collaborating closely with senior leadership, the ECM Leader empowers the team with tools, training, and resources necessary for success while upholding the standards of excellence in care management.
Comprehensive Assessments and Care Management Plans (CMPs), and acting as a clinical resource to the Alameda Care Team.
Essential Functions:
- Conducts clinical consultations for members’ care management.
- Evaluates, adjusts, and advises on member CMPs.
- Reviews assessments and communicates clinical recommendations.
- Monitors interventions/consultations to ensure proper follow up care and documentation has been completed.
- Providing clinical support and oversight when a team member identifies changes or issues in the members' condition.
- Reviews and approves Comprehensive Assessments.
- Detects and assists in reporting medication errors, duplicate doses, and possible adverse drug reactions.
- Facilitates access to primary care and behavioral health providers, as needed.
- Assists with New Hire Orientation training.
- Assists with establishing policies and procedures by providing clinical input.
- Completes administrative duties such as answering phone calls, emails, messages, updating calendars, scheduling appointments, data entry, creating reports, attending meetings, etc.
- Understands and abides by all departmental and companywide policies and procedures.
- Attends weekly meetings to provide clinical input.
- Complies with all safety and injury prevention policies and regulations.
- Performs other duties as assigned or required.
Qualifications:
- Education: BSN Required
- Experience: 2 years’ experience as an RN working in a healthcare setting, such as a medical clinic or acute care hospital.
- Experience with an EHR/EPM; eClinical Works (ECW) experience preferred.
- Experience working in an internal medicine clinic preferred.
- Pediatric Care Management experience (preferred).
- Language: Fluent in English (written and verbal). Bilingual preferred.
- License/certification: Valid, unrestricted state RN license.
- Valid BLS certification from the American Heart Association Preferred.
- Valid CPR Certification Preferred.
- Skills required:
- Competent with computers, email, virtual platforms, Excel and other Microsoft Office based programs.
- Excellent verbal and written communication skills, including the ability to convey and exchange information in a clear, effective manner.
- Ability to identify problems and use logic and related information to develop and implement solutions.
- Ability to work independently and carry out assignments to completion within the parameters of established policies and procedures.
Physical Requirements: Physical demands associated with office work:
- Consistently operating office equipment such as a telephone and computer.
- Must be able to remain in a stationary position: Remote Work
- Frequently moving or carrying office equipment weighing up to 15 pounds.
- Ability to travel to meetings and different office locations as required or requested.
