The case manager will provider case management in coordination with the clinical team for clients with substance use disorders. Some of the essential job functions are as follows:
- Serve as point of access for Medicaid Services
- Determine functional eligibility for the services and preauthorization by completing the assessment process
- Develop the client's plan of care and update as needed
- Ensure the client's plan of care is cost effective and meet their basic health, safety, and welfare needs
- Ensure clients have full and unbiased access to a variety of services and service providers to meet their specific needs
- Advocate for clients by arranging for services with individuals, businesses, and agencies for the best available service within limited resources
- Complete accurate and timely documentation
- Identify, coordinate, and deliver the five core services of independent living to clients including individual and systems advocacy, information and referral services, peer counseling, and independent living skills training
Prefer at least 1 yr. of experience in case management or other human services field
Must have a bachelor of arts or science degree in a behavioral science. Must possess a 12hr. certification for TCM or attend a 12 hr training paid for by employer.
