Our Adult Foster Care program is seeking a compassionate LPN to provide nursing case management for each member on an assigned caseload, including record maintenance and health status monitoring, to ensure that all needed Adult Foster Care (AFC) services are properly delivered.
ESSENTIAL FUNCTIONS:
- Coordinate and complete all applicable clinical assessments and evaluations.
- Develop and review on an on-going basis each member’s AFC Plan of Care.
- Select, train, evaluate and supervise caregivers in conjunction with the Care Manager.
- Conduct on-site visits with each member at the qualified setting as regulated.
- Submit semi-annual health status report to PCP.
- Review PCP summary forms and report any and all changes in health status to member’s PCP.
- Monitor the health status of all AFC members to ensure that all needs are properly delivered.
- Review AFC Caregiver logs at a minimum of every 30 days.
- Conduct orientation for each new AFC caregiver and provide ongoing training on health and aging.
- Complete nursing progress notes corresponding with each on-site visit, encounter or as member condition warrants.
- Facilitate discharges and transition from AFC program.
- Perform other duties as assigned.
- Participate in all staff meetings and required training sessions, including in-service training, as scheduled.
- Maintain all program and Agency required trainings.
Qualifications:
- Must be licensed by the Massachusetts Board of Registered Nurses.
- Ability to maintain on file a current copy of all required professional licenses, CPR, first aide and health records.
- Skill in communicating and working both independently and within a team.
- A qualifying criminal background check, driving record and a valid driver’s license required.
