Holliday's Helping Hands is looking for a Full-Time Case Managers to staff our Downey Recuperative Care facility in Downey, CA and split time at another facility in Los Angeles, CA.
An ideal candidate will have experience in the case management field working with clients from homeless population or clients that have recently been released from incarceration. Selected candidates will not only have case manager experience but also be proficient in Care Assessments and Case Notes. Case Managers work in a collaborative effort with clinician staff and health staff to provide the highest quality provision of comprehensive direct individual and group case management services while fostering teamwork among staff from multidisciplinary backgrounds. Our goal is to assist our clients in their "Journey to Home". Knowledge of ideology "Harm Reduction" and "Housing First" preferred but not required to apply.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Performs a combination, but not necessarily all, of the following duties:
- Provide individual intensive case management and outreach based on the needs of the patients;
- In collaboration with patients and the HHH team, evaluate needs of patients and development treatment/service plans accordingly;
- Develop effective, trusting relationships with patients, with a focus on facilitating independence, empowerment, and improved physical and mental health;
- Assist in involving patients in daily activities and scheduled appointments, budgeting, facilitating socialization, health and housing maintenance, community integration, and achieving life recovery;
- Develop and maintain complete, accurate, and current patient records in electronic databases as required by HHH and county standards;
- Provide case updates as requested by supervisor and in HHH team meetings;
- Provide input and evaluation of programming as a participant of HHH’s quality improvement program;
- Ensure that mental health services are reflective of HHH model which emphasizes customer choice, harm reduction, and psycho-social rehabilitation;
- Cooperatively and cohesively work with other patients of the multidisciplinary team and HHH departments, including active participation in department and agency staff meetings and trainings;
- Collaborate in the development and provision of clinical trainings;
- Complete Adult Initial Assessment, Discharges, and Progress Notes on time and within industry, DHS, ODR, and HHH standards of practice without exception;
- Meet productivity goal of a minimum of 65% of work time in direct service provision;
- Assist in providing clinical information and support to the HHH treatment team;
- Follow all COVID policies including cleaning, screening, and other protocols
- Follow all company and government policies relevant to HHH operations;
- Work proactively and cooperatively with co-workers, staff, and management;
- Attend staff meetings, trainings, huddles, and other meetings as requested by HHH;
- Other duties as assigned.
QUALIFICATIONS

Education & Experience
- High school diploma and AA in the social service field required; Bachelor Degree in relevant field desired;
- Three years’ experience working with marginalized populations;
- Timely and accurate charting skills;
- Previous experience in healthcare preferred but not required; and
- Conflict management and/or de-escalation experience required;
Licensure/Certification - Employees are responsible for maintaining individual certifications as required by job function or by law and provide verification and recertification when requested by management:
- Current, valid California Driver License with an acceptable driving record; and
- Current CPR/First Aid certification.
Language Skills
Ability to read, analyze and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public. Full English fluency (read/write/speak) is required. Bilingual Spanish skills desired but not required.
Reasoning Ability
Ability to define problems collects data, establish facts and draw valid conclusions. Synthesize complex or diverse information. Ability to interpret an extensive variety of technical instructions in mathematical or diagram from and deal with several abstract and concrete variables. Ability to interpret situations, characteristics, behavior, etc. and respond appropriately. Ability to learn and understand the function of ICMS, Project 180, and other relevant programs.
Computer Skills
To perform this job successfully, an individual should have strong knowledge of MS Outlook, MS Excel, MS Word, and Internet software
