The Case Manager position directly supports individuals with disabilities served in HOPe’s Career & Community Access Program, including implementing members’ programs and objectives in-house and in the community and documenting their progress. The Case Manager is the primary advocate responsible for the case management of approximately 8 to 10 members.
The following represents a list of essential duties and responsibilities; other duties may be assigned as required.
Assist individuals in identifying, developing, and participating in meaningful activities to increase functional independence
Support and encourage individuals as appropriate, in making and expressing choices to build on their strengths and interests, throughout their day
Identify and provide the support members need to successfully become a part of the community and participate in activities according to each person’s needs, abilities, and wishes.
Support members on their pathway to employment, preparing them for JVS services.
Implement weekly schedules and activities
Assist members physically as necessary, including, but not limited to:
Using the restroom
Regularly lift and/or transfer/move members in and out of wheelchairs, vehicles, up and down to a mat, etc.
Record progress against members’ goals
Report any unusual incidents or behaviors to appropriate supervisory personnel and complete incident reports as necessary.
Serve as the primary organizational contact for caseload, including among members’ parents/guardians, service coordinators, and service providers.
Administrative duties may be assigned on a rotational basis including Monthly Van Inspections, Monthly Facility Inspections, Human Rights Committee, Fire Safety Office

Current license, certification or registration
Effective communication skills and ability to interact with members, parents & guardians, DDS Coordinators, and others on the member’s support team.
Ability to write case notes and reports.
