Stride is a unique Individualized Intensive Program (IIP). We work with clients coming out of higher level of care (residential, wilderness, PHP, IOP) as well as those striving to avoid higher level of care to assist them in obtaining independence and autonomy in their desired community with comprehensive and wrap-around support services.
Our team supports complex cases, providing every client with a creative, personalized program to enhance quality of life and increase autonomy. Each master's level clinician partners closely with their client, the client’s family, and their community to foster recovery, growth, and stability. Our program is unique in that we encompass the whole system into treatment and provide weekly parent coaching along the way.
Role Description
The Long Term Care Care Manager Associate (LTC CMA) supports the Long Term Care team, with oversight from Care Managers (CMs) regarding specific Stride clients. You’re a great fit for this role at Stride Denver if you’re excited to work on complex case presentations while providing administrative support and integrate out-of-the-box creativity with LTC patient care plans and their staff. You will thrive in this role if you can demonstrate strong organizational skills, systems management, and clinical insights in order to support client goals and address family systems challenges. This differs from "typical" therapeutic support, as the LTC CMA is expected to work with the LTC team, therapeutic support staffs, and clients by creating and maintaining therapeutic home systems to support social skills development, executive skills, ADLs, academics, vocation, finances, and medical complexities.

Job/Clinical Duties:
- Support CM in client care management
- Engage in 1:1 sessions with client, following up on CM therapeutic goals
- Crisis intervention support
- Participate in weekly staff clinicals
- Model appropriate and desirable behaviors
- Develop and implement treatment plans
- Assist in goal setting and the achievement process
- Implement action-based interventions
- Support implementation of personalized home-assistive processes
- Produce daily HIPAA-compliant clinical documentation
- Supervise support staff as appropriate
- Work independently, accept feedback, and take initiative with client and family needs
Position Requirements:
- Pursuit of Master's Degree in a mental health related field
- Minimum of 1-2 years working in a clinical setting
- Strong verbal and written communication skills
- Drug test and background check
- Valid drivers license and proof of auto insurance
- Dependable & strong time management skills
- Ability to establish appropriate boundaries with clients, families, and co-workers
- Appropriate use of clinical language and presentation in a clinical setting and with a multidisciplinary team
- Function well in a team/collaborative setting, uphold strong moral values, and maintain a high degree of confidentiality
