SUMMARY: Join our dedicated team at the Native American Community Clinic (NACC) as a Community Health Nurse. In this role, you will play a crucial part in enhancing the health and well-being of our community through patient outreach, education, and support for various health initiatives and quality improvement projects.
DUTIES AND RESPONSIBILITIES:
- Patient Outreach and Coordination:
- Conduct phone and mail outreach to schedule appointments and coordinate basic care.
- Generate and analyze health reports to support preventative health and chronic disease management.
- Educate patients on preventative care timelines and follow-up for screenings, including cancer, diabetes, cardiovascular disease, and well-child checks.
- Recruit participants for chronic disease management groups and preventative care events.
- Program Support and Evaluation:
- Manage referrals for Community Health Department programs, including diabetes education, prevention programs, and mobile medical services.
- Assist in evaluating program effectiveness and implementing improvements to meet grant deliverables.
- Track attendance and engagement metrics for various programs and compile monthly activity reports.
- Support grant-specific reporting and documentation as requested by the Community Health Manager.
- Administrative and Collaborative Duties:
- Assist with budget maintenance and purchasing for health programs.
- Collaborate with grant/project evaluators to develop required data and reports.
- Maintain regular communication with project partners and fulfill grant requirements, including submitting reports.
- Contribute to sustainability planning for ongoing Community Health Programs.
- Actively participate in the outreach team and support additional duties as assigned.
- Cancer Screening Initiatives:
- Identify and assist patients due for breast, cervical, and colorectal cancer screenings.
- Provide education and follow-up for cancer screenings and related events.
- Complete enrollment forms and track patient progress in cancer screening programs.
- Diabetes Care Outreach:
- Conduct outreach calls for patients due for diabetes follow-up appointments.
- Identify and document barriers to care for diabetes patients.
- Lung Cancer Screening Promotion:
- Identify eligible patients for lung cancer screening and deliver promotional education through mailings and phone calls.
QUALIFICATIONS:
- Active Minnesota RN or LPN license.
- Strong communication and organizational skills.
- Ability to work collaboratively with a diverse team.
- Passion for improving community health and well-being.
- Experience in health outreach, education, or related fields is a plus.
- Experience in the Native American community, preferred.
- At least two years of work experience in community health preferred
SUPERVISORY RESPONSIBILITIES:
This job has no supervisory responsibilities.
Job Type: Full-time
Pay: $30.00 - $41.20 per hour
